It is in this category that the consequences of "error" fall, which is also too easily called "medical error", implying once again that the physician is solely responsible. The more appropriate term seems to be "care error", since care does not only consist of a medical prescription or an act that is properlymedical prescription or a strictly medical act, but a set of actions and behaviors of actors, most often multiple, in various structures with organizational conditions that are always singular.From this reflection, the National Academy of Medicine proposed a new approach to medical error, considered as a systemic failure, for which prevention, rather than punishment, is likely to reduce undesirable events related to care.
